Actiunea farmacologica: Valaciclovirul este valil-esterul aciclovirului si poate fi considerat ca un promedicament, deoarece in organism se transforma in aciclovir. Se absoarbe rapid si totalmente. Aciclovirul este un analog aciclic sintetic al unui nucleozid purinic. Pentru a deveni activ trebuie supus fosforilarii prin intermediul timidin-kinazei. Monofosfatul este transformat ulterior in difosfat de guanilat-kinaza, apoi in trifosfat de o serie de enzime celulare. Aciclovirul-trifosfat interfera cu ADN polimeraza virala si inhiba replicarea ADN-ului viral. Este activ fata de Herpes simplex tip 1 (HSV-1) si 2 (HSV-2), Varicella zoster (VZV), Epstein-Barr (EBV) si citomegalovirus (CMV). Virusurile sunt enumerate in ordinea descrescinda a sensibilitatii lor la aciclovir. Aciclovirul este putin toxic pentru celulele neinfectate din cauza ca 1) acestea il utilizeaza intr-o masura mai mica, 2) il transforma mai putin in forma activa si 3) ADN-polimeraza umana este mai putin sensibila la forma lui activa.
INDICATII
Herpes zoster asociat cu dureri accentuate, nevralgia postherpetica, herpesul simplu cutanat sau al mucoaselor, herpesul genital, stadiile precoce ale HIV infectiei.
DOZE SI MOD DE ADMINISTRARE
In tratamentul herpesului zoster cite 1.000 mg de 3 ori pe zi timp de 7 zile. In herpesul simplu cite 2000 mg (4 tab.) de 2 ori pe zi (12 h diferenta de adm. ).
CONTRAINDICATII
Hipersensibilitatea la preparat.
