PROPRIETATILE FARMACOLOGICE
Lansoprazolul, derivat de benzimidazol, un agent antisecretor si antiulceros, inhiba selectiv pompa de protoni a celulelor parietale gastrice si secretia de acid clorhidric, atit bazala, cit si stimulata. Nu modifica secretia pepsinei si nici motilitatea gastrica. Este activ contra Helicobacter pylori datorita inhibitiei ureazei acestei bacterii, insa pentru eradicarea infectiei trebuie utilizat in combinatie cu antibiotice.
INDICATII
Ulcerul gastric si duodenal, reflux-esofagita, sindromul Zollinger-Ellison.
Utilizare terapeutica :
In ulcerul duodenal cite 30 mg o data pe zi timp de 4 saptamini inainte de masa. In ulcerul gastric cite 30 mg o data pe zi timp de 4 saptamini, la necesitate pina la 6-8 saptamini. In reflux-esofagita (esoagita eroziva) cite 30 mg o data pe zi timp de 4-8 saptamini, in unele cazuri plus inca 8 saptamini. In sindromul Zollinger-Ellison doza initiala este de cite 60 mg o data pe zi, apoi crescuta pina la 120-180 mg/zi. Daca doza depaseste 90 mg/zi, se va administra in 2 prize egale, dimineata si seara. Pentru eradicarea infectiei cu Helicobacter pylori se recomanda triterapie sau biterapie. In triterapie: Lansoprazol cite 30 mg de 2 ori pe zi inainte de sau dupa mese impreuna cu claritromicina 500 mg de 2 ori pe zi si amoxicilina cite 1000 mg de 2 ori pe zi timp de 14 zile. In biterapie: Lansoprazol cite 30 mg de 2 ori pe zi si claritromicina cite 500 mg de 3 ori pe zi, la intervale de 8 ore, timp de 14 zile. In ambele scheme dupa aceasta perioada de 14 zile terapia cu lansoprazol va continua inca 2 saptamini, necesare pentru cicatrizarea ulcerului.
CONTRAINDICATII
Hipersensibilitatea la preparat.
INTERACTIUNI CU ALTE MEDICAMENTE
Lansoprazolul creste clearance-ul teofilinei si doza acesteia trebuie ajustata. Sucralfatul scade absorbtia lansoprazolului si acesta trebuie administrat cu 30 min inainte de sucralfat. Reduce absorbtia ketoconazolului, esterilor ampicilinei, sarurilor dr fier si a digoxinei.
ATENTIONARI SI PRECAUTII SPECIALE DE UTILIZARE
In timpul graviditatii poate fi utilizat numai daca este absolut necesar. In insuficienta sau ciroza hepatica doza nu trebuie sa depaseasca 30 mg/zi.
